If this is life-threatening, call 112 (national emergency) or 119 (medical emergency / ambulance) first. 112 is the national emergency number in Indonesia; 119 is the public health emergency line (PSC 119) for ambulance and medical dispatch. Make that call before you contact us. Once help is moving, message the desk and we take over the coordination around it.
Why escalation is normal, not a failure
A visit is an assessment. One legitimate outcome of an assessment is “this needs imaging”, “this needs laboratory results”, “this needs a surgical opinion” or “this should be observed somewhere with monitoring”. That is the visit doing its job — narrowing the question quickly and cheaply. Treating escalation as a sign something went wrong leads families to resist it, which is the actual danger.
Who decides
The practitioner who has examined the patient. Not the family, not a concierge desk, not a page on a website. If they say a facility is needed, the useful response is to ask what kind of facility and how urgently, and then to move. We do not second-guess that judgement and we have no standing to.
The two questions to ask before you leave
First: what does the patient need at the facility — which department, which investigation, which specialty? That single answer determines the destination, and it is far better coming from the practitioner who has just examined the patient than reconstructed later. Second: how urgent is it — now, tonight, or in the morning? The answer changes the transport and whether an emergency department is the right door.
What we do in the handover
Take the practitioner’s answer to those two questions and confirm by phone, in Bahasa Indonesia, which facility can receive that case now — which is not always the nearest, because a department may be full or an investigation unavailable overnight. Arrange the vehicle. Ask the attending practitioner for a written note and carry it with the patient. Call ahead so the receiving team has the outline. Then interpret at the counter, handle intake and any deposit, and start the insurer correspondence.
The thing most often lost
The note. A patient arriving at a facility with nothing written from the practitioner who just saw them gets assessed from scratch, which wastes the visit entirely. Ask for it, in English where possible, and make sure it physically travels with the patient rather than being promised by email.
When to skip the visit altogether
If someone is unconscious or difficult to rouse, not breathing normally, bleeding heavily, seizing, showing signs of a stroke or heart attack, has severe chest pain, has taken an overdose, or has been in a serious crash, do not arrange a visit. Dial 112 or 119 immediately and message us afterwards. Arranging a house call in that situation is the most expensive mistake available in this whole sequence.
Family logistics
The part that gets forgotten in the first hour. If one adult goes with the patient, someone still has to handle children, the accommodation, and relatives abroad who need to be told something accurate rather than alarming. We arrange transport for the rest of the party and keep one thread going so nobody is reconstructing the story later.
We publish a directory of listed facilities that carries its source and the date each entry was checked, and we withhold any direct line that has not passed two independent sources. Capability still changes by the hour, so while a case is live we confirm by phone, in Bahasa Indonesia, instead of relying on what a page said yesterday.
Picking the right destination is in how to choose the right hospital in Bali, and what happens at the counter in hospital admission and discharge paperwork.

Do not argue with an escalation
It is a common and understandable instinct: the practitioner has said a facility is needed, the patient feels a little better, and it is late. Resisting at that point is the most consequential mistake in this sequence, because the recommendation was made by the only person who has actually examined the patient. If the logistics feel impossible, say so and let us solve them — that is a solvable problem in a way that the clinical question is not.
Sanur Emergency is an independent medical concierge. We are not a clinic, hospital or ambulance operator, we do not employ medical staff, and we are not affiliated with any medical facility. All clinical care is delivered by licensed providers.
